Use this link to self screen yourself and your player before ice time to help save time prior to practices and games:

This will also be emailed to all registration emails we have.And hopefully posted in the website.

We are in the process of getting a screening app but it is not ready at this time.

You may see an email come from the website/Shelby that talks about setting up your Sportsheadz account. You can go ahead and set up your account as well as download the Sportsheads app from the app store & google play store. More updates about the app will be posted soon, but in the mean time continue to use this google form. Thanks!
